All New Patients at Cahaba Valley Surgical Group, PC are
required to complete paperwork so that we accomplish the following:
- Accurately diagnose and treat you with the best
possible care
- Coordinate services with the hospital and your
referring doctor
- File health care claims and coordinate payment on your
behalf
