New Patient Forms

All New Patients at Cahaba Valley Surgical Group, PC are required to complete paperwork so that we accomplish the following:

1. Accurately diagnose and treat you with the best possible care
2. Coordinate services with the hospital and your referring doctor
3. File health care claims and coordinate payment on your behalf

While the following two forms do not represent all of the paperwork a New Patient
must complete at their first visit, it does represent the majority of what we need to
collect.  Completing this information will shorten your processing time at your first visit.

Please note that if you are seeing us for our Bariatric Program (gastric bypass or laparoscopic adjustable gastric banding) we have additional requirements that can
be found in that section of this site. the application materials for weight loss surgery
are different from those below and can be found through the Bariatric Program
section of the website.
