Our office is located in the Shelby Medical Plaza, on the Shelby Baptist Medical Center Campus.  The Medical Plaza is located between the hospital and the Diagnostic Center on the main campus.  We are on the second floor.

From the North (Huntsville/Tuscaloosa Area):

Take I-65 South towards Birmingham. Continue through the city of Birmingham approximately 12 miles heading towards Montgomery to Exit 242 (Pelham Exit). You will see large Gas Tanks on the right hand side as you exit. At the bottom of the exit turn RIGHT. This is County Road 52 West. Go 6 tenths of a mile straight through 1 traffic light. At the 2nd traffic light, turn LEFT onto Highway 31 South. Follow Highway 31 South for 2.1 miles. Pass the 1022 Tower building at the 3rd traffic light for Industrial Road. Continue straight through the light. You will see the Shelby Medical Plaza on the left hand side of the street, actually connected to the Shelby Baptist Medical Center. Parking is located to the left and front of the building entrance. Cahaba Valley Surgical Group, P.C. is located on the second floor, Suite 250 of this building.

If you are coming from Tuscaloosa, take I-459 South to junction for I-65 South towards Montgomery. Take Exit 242 (Pelham Exit). You will see large Gas Tanks on the right hand side as you exit. At the bottom of the exit turn RIGHT. This is County Road 52 West. Go 6 tenths of a mile straight through one traffic light. At the 2nd traffic light, turn LEFT onto Highway 31 South. Follow Highway 31 South for 2.1 miles. Pass a new professional medical building at the 3rd traffic light for Industrial Road. Continue straight through the light. You will see the Shelby Medical Plaza on the left hand side of the street, actually connected to the Shelby Baptist Medical Center. Parking is located to the left and front of the building entrance. From the South (Montgomery Area):

Take I-65 North towards Birmingham. Take Exit 238 (Alabaster Exit). Turn LEFT off the exit ramp. That is Highway 31 North. Follow Highway 31 North for 1.8 miles. You will go through 2 traffic lights straight. On the right hand side of the street, you will see the Shelby Baptist Medical Center. Pass the entrance for the hospital emergency room (there's an Arby's at this entrance) and take the very next RIGHT into the parking lot for the Shelby Medical Plaza. Parking is located to the left and front of the building entrance. Cahaba Valley Surgical Group, P.C. is located on the second floor, Suite 250 of this building.

From the East (Gadsden/Atlanta Area):

If you are coming from the Gadsden/Ft. Payne area, take I-59 South towards Birmingham, Follow signs for I-459 towards Atlanta and Montgomery. Take I-459 South heading towards Montgomery to the junction of I-65 South. Take I-65 South towards Montgomery to Exit 242 (Pelham Exit). You will see large Gas Tanks on the right hand side as you exit. At the bottom of the exit turn RIGHT. This is County Road 52 West. Go 6 tenths of a mile straight through one traffic light. At the 2nd traffic light, turn LEFT onto Highway 31 South. Follow Highway 31 South for 2.1 miles. Pass a new professional medical building at the 3rd traffic light for Industrial Road. If you are coming from Atlanta, take I-20 West towards Birmingham to the junction of I-459 South. Take I-459 South towards Montgomery to the junction of I-65 South. Take I-65 South towards Montgomery to Exit 242 (Pelham Exit). You will see large Gas Tanks on the right hand side as you exit. At the bottom of the exit turn RIGHT. This is County Road 52 West. Go 6 tenths of a mile straight through one traffic light. At the 2nd traffic light, turn LEFT onto Highway 31 South. Follow Highway 31 South for 2.1 miles. From the West (Sylacauga Area):

Take Highway 280 East towards Birmingham. At the intersection for Highway 119, turn LEFT. Follow Highway 119 South which is also Cahaba Valley Road to the intersection for I-65 South, approximately 10 miles. You will pass the Indian Springs School on the left before getting to the entrance for I-65 South. Take I-65 South towards Montgomery to Exit 242(Pelham Exit). You will see large Gas Tanks on the right hand side as you exit. At the bottom of the exit turn RIGHT. This is County Road 52 West. Go 6 tenths of a mile straight through one traffic light. At the 2nd traffic light, turn LEFT onto Highway 31 South. Follow Highway 31 South for 2.1 miles. Pass a new professional medical building at the 3rd traffic light for Industrial Road.
