Evaluation by Our Surgeons
In the initial letter we send you
in response to receiving your packet, we will outline some steps you need to
take prior to scheduling an initial evaluation in our office. We recommend
obtaining relevant medical records from your other doctor(s) which show evidence
of your weight over a period of time (for example, 3 years), and any medical
conditions related to your weight. Your insurance company may also require a
nutritional consultation or psychiatric consultation. We will advise you how to
go about that, if it is necessary. Although Cahaba Valley Surgical Group staff
will ultimately arrange your information into a packet and submit to your
insurance company for medical review, it is your responsibility to obtain the
requested documents from your other care providers (primary physicians,
dieticians, psychiatric professionals, etc.). When you have obtained all
relevant materials, please submit them together to our Bariatric Coordinator for
review. This review will take approximately 15 - 30 business days from our receipt of
this packet. We will then contact you to discuss your information, and schedule
your initial consult with one of our surgeons. We are not able to submit your
information to your insurance company until we have conducted an evaluation.
We do not schedule bariatric evaluations for patients for whom we have not
received all requested clinical data, who do not meet our baseline criteria, or
who have no history of weight loss attempts through non-surgical methods.
