Please read our New Patient Information and insurance policies before your first appointment.

Please download and complete our New Patient Forms prior to your first office visit. Please note your previous surgeries and any medications you are taking.

We are open Monday to Friday (except Holidays) between 8:00am to 4:30pm.

As a Specialist clinic, in most cases patients need referral from their General Practitioner, family physician or other doctor in order to be covered by insurance.

When you come for your appointment, remember to bring the following:

• Drivers License
• Insurance Card(s)
• Copies of operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits.

To cancel an appointment:

Telephone the office during business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
